Cardiovascular program

- Cardiac assessment by RNs with competencies in cardiac assessment
- Cardiac education for patient/family
- Post-acute care CHF, acute MI, post CABG, post-valve replacement, endocarditis and cardiomyopathy
- Diet instruction/modification
- Medication teaching, monitoring of medication changes and anticoagulant monitoring
- Dressing changes, infusion therapy, venipuncture for lab monitoring of unstable conditions and medication changes
- X-rays and Holter monitoring in home in conjunction with the Nursing Home Group
- In-home simple spirometry teaching
- Single lead and 12-lead EKGs by RN staff
- Pulse oximetry every visit until stable

- RN case managers with cardiac/critical care experience
- All nursing staff BCLS certified
- Cardiovascular nursing staff with training and competency testing in basic EKG and cardiac drugs
- Registered dietician available for consultation
- PICC line-certified RN, certified infusion therapy nurse
- Nursing supervisor/program manager with six years experience as cardiac nurse
